I have read and agree to abide by the Fort Atkinson Orchestra behavior guidelines while participating in any function relative to the Orchestra. If I break this agreement or my conduct is not satisfactory to the chaperone(s), the orchestra director, or the hosting organization/facility, I understand that I may be sent home and will be responsible for paying all costs incurred by the early departure.

I understand that I may be asked to forfeit all funds expended by
behalf during the event.

I understand that I may be ineligible to participate in future Orchestra out of school activities.

I, as parent or guardian, understand the above rules and penalties, accept them,
